Sam "Ace" Rothstein is a notable gambling handicapper. When the Chicago mob gains control of the Tangiers Casino in Las Vegas, they hire Ace to run the place. Ace brings along old friend Billy Sherbert (Don Rickles) to work as the casino manager.
Meanwhile, the mob bosses hire Nicky Santoro, Ace's childhood friend to watch over the casino and make sure their plans to skim money from the casino go off the way they wanted.
While Ace was a level-headed patient type, Nicky was a hot-headed psychopath who reveled in hurting people, such as stabbing a man repeatedly in the neck with a pen, popping a guys eyes out, sticking ice picks in a guys balls, and beating Billy Sherbert ("You bald-headed Jew prick!") over the head with a telephone.
If you don’t have my money for me, I’ll crack your fucking head in front of everyone in the bank. And just about the time I get out of jail, hopefully, you’ll be coming out of your coma. But guess what? I’ll crack your fucking head again! ‘Cause I’m fucking stupid! I don’t give a fuck about jail! That’s my business. That’s what I do” - Nicky
Recalling Nicky’s method of operation, Ace Rothstein says, “No matter how big a guy was, Nicky would take him on. You beat Nicky with fists, he comes back with a bat; you beat him with a knife, he comes back with a gun; and if you beat him with a gun, you better kill 'em, ‘cause he'll be coming back and back, until one of you is dead.”
Once, a guy wearing cowboy boots, removed his boots and propped his sock covered feet on the playing tables. Ace found it very distasteful and ordered the man removed from the casino, not knowing he was one of Nicky's associates. The cowboy complained to Nicky who called Ace and asked permission to let his friend back into the casino. But when Nicky discovered the cowboy had insulted Ace, Nicky turned on his friend, and shouted. "You took your boots off? You put your feet on the table...you shit-kicking, stinky, horse-manure-smelling motherfucker you! You fuck me up over there, I'll stick you in a hole in the fucking desert! You understand? If you ever get of line over there again, I'll smash your fuckin' head in so hard, you won't be able to put that cowboy hat on. You hear me? Fuckin' hick."
While Nicky was certainly the muscle, Ace was no push over. When he felt someone should be disciplined for offending the casino, he had no qualms instructing his co-workers to smash a hand with a hammer to make a point after two guys are caught cheating by signaling one another to win at cards.

|Ace Rothstein:||[to the winner] All right, I'm gonna give you a choice. You can either have the money and the hammer or you can walk out of here. You can't have both. What do you want?|
|Winner:||I just wanna get outta here.|
|Ace Rothstein:||And don't forget to tell your friends what happens if they fuck up around here. You understand?|
|Winner:||I'm sorry. I made a bad mistake.|
|Ace Rothstein:||You're fuckin' right you made a bad mistake. 'Cause if you come back here, if we catch either one of ya, we're gonna break your fuckin' heads and you won't walk out of here. You see that fuckin' saw? We're gonna use it. We don't fuck around in this place. You got it? Get outta here.|
|Winner:||[relieved] Thank you.|
|Ace Rothstein:||[to Security escorts about the broken hand] Throw him out into the alley and just tell the cops he got hit by a car.|
Besides having to deal with Nicky's nutty nature, and other problems that come with running a casino, Ace also had the headache of handling his alcoholic wife, Ginger (Sharon Stone), who once tied their daughter, Amy to a bedpost while she left the house for a few hours.
After cheating on Ace with Nicky, asking Nicky to kill Ace, and threatening to go to the FBI, Ginger leaves, but not before emptying Ace's safe-deposit box of all his money. A short time later, Ginger is found dead from an overdose of cocaine (Ace suspected it was a "hot-dose" administered to keep her quiet.)
"They had enough of Nicky. They had enough. I mean, how much more were they gonna take? So, they made an example of him and his brother: they buried them while they were still breathing." - Ace
After the Tangiers gets too hot and the authorities take over the casino, the mob bosses begin to kill off anyone who could testify against their skimming operations. Nicky and his brother were taken to the desert and beaten with baseball bats.
In the end, Ace survives a car bombing (Nicky's work) and goes back East to pick up his former career as gambling handicapper.
"The town will never be the same. After the Tangiers, the big corporations took it all over. Today it looks like Disneyland. And while the kids play cardboard pirates, Mommy and Daddy drop the house payments and Junior's college money on the poker slots. In the old days, dealers knew your name, what you drank, what you played. Today, it's like checkin' into an airport. And if you order room service, you're lucky if you get it by Thursday. Today, it's all gone. You get a whale show up with four million in a suitcase, and some twenty-five-year-old hotel school kid is gonna want his Social Security Number. After the Teamsters got knocked out of the box, the corporations tore down practically every one of the old casinos. And where did the money come from to rebuild the pyramids? Junk bonds. But in the end, I wound up right back where I started. I could still pick winners, and I could still make money for all kinds of people back home. And why mess up a good thing?" - Ace [last lines]
